Area Agency on Aging of Northwest Arkansas, Fayetteville
The Area Agency on Aging (AAA) of Northwest Arkansas is committed to improving the quality of care for the Medicare beneficiaries in its region. It has partnered with many health care organizations from Fayetteville, Harrison, Mountain Home and everywhere in between. This health care improvement group, titled the "Coalition for the Ozarks," seeks to improve the quality of care to their patients by improving care transitions.

East Arkansas Area Agency on Aging, Jonesboro
In order to improve care transitions for the Medicare beneficiaries in its region, the East Arkansas Area Agency on Aging (AAA) formed a health care organization titled the "Northeast Arkansas Coalition." The goal of this coalition is to improve the patient experience as they transition among health care settings.

Carelink, Little Rock
CareLink, the Area Agency on Aging (AAA) for central Arkansas, is focused on bringing organizations together to better the quality of care for their Medicare beneficiaries. The "Central Arkansas Care Transitions Coalition" came together with a common goal of improving the quality of patient care. Its coalition was accepted into the Community-Based Care Transitions Program (CCTP), sanctioned through the Affordable Care Act and will receive funding for their services. Its aim is to provide successful health care transitions for the patients it serves.
